Angela R. Fields is a nationally recognized Executive Community Health Worker (CHW), consultant, and advocate with over 30 years of experience in healthcare, mental health, addiction services, and community-based systems. As the Founder and Director of Community Health Worker Concepts LLC, Angela leads efforts to strengthen and sustain the CHW workforce through culturally responsive training, strategic consulting, and policy advocacy.

A lifelong champion for health equity, Angela’s work focuses on uplifting underserved communities and building cross-sector solutions that reflect lived experience and community voice. Her career spans frontline service delivery, nonprofit leadership, curriculum development, and statewide workforce initiatives.

Angela authored the revised Minnesota Standardized CHW Certificate Mental Health Curriculum, now used to train CHWs across the state. She has also served as the Associate Executive Director of a statewide nonprofit, and as Student Services Coordinator for the Minnesota CHW Certificate Program, where she mentored future CHWs and led statewide peer learning and advocacy efforts.

With deep expertise in addiction treatment, harm reduction, and mental health, Angela has developed programs that increase equitable access to care—especially for communities of color, rural populations, and low-income families impacted by the opioid crisis and systemic disparities. Her work is grounded in culturally responsive approaches, compassion, and health justice.

Angela regularly collaborates with multidisciplinary teams, manages CHW-led grant initiatives, and leads legislative engagement to elevate the CHW profession at the local and state levels.

Outside of her professional work, Angela enjoys gardening, cooking, and spending time with her children and grandchildren—activities that reflect her deep commitment to wellness, family, and community.
